GUITAR LESSON AGREEMENT
INSTRUCTOR – VINCENT A. MELLONE
Fees are currently $140 per month ($35.00 per lesson) for four one-half hour lessons. If there are five lessons in the month, the fee is $175 per month. Fees are payable at the first lesson of the month.
Fees are payable in full at the first lesson of each month. If you miss the first lesson of the month or forget payment, please bring your payment to the store during the first week or mail to my address listed below. Non-payment beyond the second week of the month may result in forfeiture of the spot.
There will be no lessons on the following days: Fourth of July, Thanksgiving, Christmas Eve, Christmas Day, New Years Eve, and New Years Day. Please deduct $35 (one lesson) from your monthly tuition if your lesson falls on one of those days.
CANCELLATION AND MISSED LESSONS will be charged in full unless 24 hours notice is given. Proper cancellations will not result in refunds and a makeup lesson will be offered during that same week, but not guaranteed. In order for the student to make progress it is important to establish a consistent lesson time, so please arrange other activities so that you can come to your lesson every week. I understand that on rare occasions, emergencies or a contagious illness may arise that will make it impossible for you to keep your scheduled lesson. In that case I will try and reschedule, but if not, you will not be charged for the missed lesson.
Prior to Summer Vacation please notify me at least two weeks in advance if you will miss any lessons.
I will give my students advance notice concerning my vacation absences. No tuition will be charged during my absences.
When monthly tuition fees are subject to change, I will provide my students with 30 days advance notice.
If these policy rules are not fully complied with, I reserve the right to terminate the contract immediately.
To stop the lessons, students/Parents must give me two weeks’ notice.
